Outpatient Registered Nurse (RN) Care Coordinator, First Shift, Comprehensive Care for Complex Conditions (Primary Care)

UC Health is hiring a full-time Outpatient RN Care Coordinator for the Primary Care offices. This position is first shift located at the Medical Arts building with University of Cincinnati Physicians Company, LLC .

The RN Care Coordinator will support the Primary Care delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures.

Job Responsibilities

The RN Care Coordinator is an individual who holds a current state license to practice nursing. The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals. The RN utilizes knowledge derived from the principles of biological, physical, behavioral, social and nursing sciences to assess, plan, implement and evaluate patient care. The care is culturally based and age-specific.
  • Collaborate with care team, interdisciplinary departments at the system level to plan, implement, and/or evaluate services.
  • Facilitate communication and coordination between members of the health care team (including the medical home and community services), involving the client in the decision-making process in order to minimize fragmentation in the services.
  • Plan with the client, family or caregiver and the provider to document a patient centered plan of care focused on achieving quality, and cost effective outcomes.
  • Gives clear instructions to patients/family regarding treatment, and involves family/guardian in the assessment, initial treatment, and continuing care of the patient.
  • Identifies and responds appropriately to different patient needs resulting from behavioral health problems, unique psychological needs or those associated with religious/cultural norms.
  • Create an environment to support patient safety by integrating patient safety goals into daily practice based of the patient's age and populations served.
  • Serve as a resource for other members of the health care team.
  • Identify improvement opportunities and communicates them to unit's management team, providing supporting data when possible.


Qualifications

Education and Experience Requirements:
  • Graduate from an accredited school of nursing
  • Bachelor of Science in Nursing, required
  • Current OH RN License
  • 2-3 Years equivalent experience - Preferred
