Responsible for ensuring that quality, patient centered Social Work services are being provided to patients on a daily basis. Ensure adequate staff coverage is available to provide comprehensive services to patients and their families; support to the multidisciplinary health care team; and intervene with problem resolution on complex cases. Identify and address internal and external barriers that prevent and/or interfere with interventions and treatment plans. Offer support and feedback to staff, as appropriate to maintain professional practice.

Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs that promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families' emotional, psychological, social, financial and developmental aspects of care.

Engagement and Exploration:

Engage patients and/ or their families into a helping process. Assess the psychosocial and mental health needs of patients and families to:
  • The supervisor will work under the guidance and supervision of the Manager Enhance patient's health status.
  • Direct all patient care services including psychosocial assessment, crisis intervention, counseling, referrals, and disposition when needed. Assist staff in prioritizing activities, locating resources, and scheduling educational engagements.
  • Recommend strategies to enhance productivity and quality of work and ensure consistent documentation to meet regulatory requirements Maintain statistical information as required for productivity reports Assist with scheduling, shift assignment, and reallocation of staff as needed to ensure coverage.
  • Adheres to the tasks and responsibilities specific to the department, service or clinic.

Goal Development and Implementation:

Develop a comprehensive plan of intervention in collaboration with the patient/ family and healthcare team. Implement a plan of care and monitor patient's response:
  • Assist in the development and implementation of departmental policies and procedures.
  • Assist with the interviewing, selection, orientation, and training of new personnel.
  • Ensure staff know performance expectations and hold them accountable for professional practice.
  • Establish and maintain a working environment that supports the Values and assist in Service Recovery Develop and maintain liaison relationships within the department, other health care professionals and community agencies.

Evaluation and Termination:
  • Briefly summarize care plan with patient/family and respond to questions and concerns.
  • Provide patient/ family with linkages to community resources and services.
  • Participate in process improvements in evaluating patient outcomes.
  • Identify barriers in service delivery systems.
  • Assess for transportation needs and arrange as appropriate.
  • Facilitate referrals to disease specific agencies, especially as it relates to employment, financial resources, support services, chemical dependency services.

Education and Consultation:
  • Provide consultation and training to medical staff and other healthcare professionals on psychosocial issues regarding illness and barriers to maximizing improved health status.
  • Collaborate with co-workers and management to ensure the on-going development of the Social Work Department.

  • Complete annual performance evaluations.
  • Attend staff meetings and mandatory departmental in-service training sessions & continuing education in-services; maintain positive working relationships with and knowledge about community agencies and services.
  • Maintain timely, clear and concise documentation; and write reports as needed.
  • Respond to E-mail requests and provide information as requested.
  • Participate in quality and safety initiatives.
  • Provide an environment that utilizes the core values of standards of practice and ethics.


Education Requirements: Master of Social Work degree from an accredited School of Social Work | Licensed Independent

Qualifications: Social Worker Supervisor by the State of Ohio Social Work Board (LISW-S) Certification preferred; or license eligible 5 years clinical experience. Supervisory experience preferred.
