UC Health is hiring a full-time Case Facilitator for the department of Employee Health And Safety

The Case Facilitator is responsible for managing all aspects of employee leave of absence and accommodation requests. This includes the administration of Family and Medical Leave Act (FMLA), non-FMLA leave, military leave, personal leave, short and long-term disability, and requests for accommodation under the Americans with Disabilities Act (ADA) and the Pregnant Workers Fairness Act (PWFA), as well as other applicable accommodation requests. Additionally, the role includes overseeing modified duty assignments. The Case Facilitator ensures compliance with applicable laws and regulations, maintains accurate records, and provides support and guidance to employees throughout the leave and accommodation process. In addition, this role will coordinate compensation for absence to include PTO, STD, LTD and Worker's Compensation in collaboration with third party vendors.

Responsibilities

  • Maintain Leave of Absence System:
    • Administer all types of leave of absence including FMLA, non-FMLA, military leave, personal leave, and short and long-term disability. Reviews application to be sure it is complete and meets the standards under FMLA law. If not complete, sends appropriate paperwork and information back to associate and /or their Healthcare provider if clarification is needed
    • Manages the leave of absence case for 200-250 employees, once the initial claims is set up; electronically makes leave plan determinations, sends all appropropriate notifications in a timely manner; fully utilizes all LeavePro functionality during the course of the leave including making determination and sending all appropriate notifications to include leave expiration and leave exhaustions, entering notes, managing and completing task list tasks in a timely manner, properly attaching documents to a case, properly executing timely return to work notifications, all from inception to resolution of the leave
    • Checks eligibility status for FMLA and short-term disability. Coordinates all eligible short term disability cases with the disability provider.
    • Enters data in LOA database, Notifies associate and Manager in writing of decision in accordance with FMLA law and UC Health policy.
    • Provides advisement and consultation to department managers related to FMLA, leave of absence and unscheduled absence related matters.
    • Maintains electronic records in Alight, in good order for all leaves; attaches all pertinent information to the case in Alight
    • Processes status changes to take associates off of work and when the associate returns from leave. Any extensions are reviewed and processed in same manner
    • Processes payroll adjustments as needed
    • Educates associates and Managers concerning FMLA and UC Health policies regarding medical leaves
    • Coordinates long-term disability applications with the disability carrier for associates on extended medical Leave. Works with the long-term disability carrier to provide all needed information.
    • Letter of notification is sent to associates on leave for 6 months to notify regarding possible termination
    • Responds to requests from outside agencies and/or attorneys to verify time off for medical reasons and lost wages
  • Workplace Accommodation Requests:
    • Reviews all requests for modified duty and various accommodation request including but not limited to ADA, PWFA and Religious Accommodations for completeness. Communicates with employee if additional information is necessary in order to make a determination.
    • Completes interactive process with the employee, manager and HR to determine if accommodations can be made
    • Collaborates and consults with Employee Health Nurse Partners as needed for complex accommodation requests.
    • Makes determinations on all accommodation requests. Notifies employee, manager and HR of determination. Enters information into the Alight database, sends status change to HRMS Manager and associate are notified of decision
    • Reviews requests for extensions to see if appropriate and reasonable
    • Answers questions from associates and managers
    • Works with Human Resources if any issues or concerns regarding light duty Associates with Permanent restrictions that cannot be accommodated will be referred to Human Resources to look for another position
  • Customer Service:
    • Listens to the needs of all customers
    • Provides information as requested or needed to help associate and Managers to understand the process
    • Follows- up on all issues
    • Seeks guidance from Director, Medical Director and/or Human Resources as needed
    • Functions as active part of team, helping out coworkers as needed
    • Participates in departmental and organizational meetings.


Qualifications

  • Minimum Required: Associates Degree.
  • Preferred: Bachelor Degree. Combination of post-secondary education and experience will be considered in lieu of degree.
  • Preferred: 1-2 years leave management experience.
