The Patient Safety Manager will have primary oversight of the facility-wide patient safety evaluation system. The Patient Safety Manager will direct other within the facility towards process improvements which support the reduction of medical/health care errors and other factors that contribute to unintended adverse patient outcomes. The Patient Safety Manager provides leadership for safety assessments, coordinates the activities of the patient safety committee, educates on the system-based causes for medical error, consults with management and staff, and communicates literature-based ideas regarding effective patient safety strategies to others within the organization.

Responsibilities

Patient Safety Data Collection and Reporting.- 50% Oversee the management and use of medical error information, including review of internal incident reports. Investigate patient safety issues within the facility. Supports and encourages incident reporting throughout the organization through a non-punitive reporting system. Make summaries and report patient safety data in usable and actionable formats. Assists when requested in collection of patient related incident information. Daily contacts with hospital staff to maintain an understanding of the hospitals patient safety practices, and to be an accessible resource to staff in matters of patient safety. Support and improve mechanisms for internal communication of patient safety related information. Coordinate the activities of the Patient Safety Committee and weekly Enterprise incident reviews.

Root Cause Analysis- 25% Coordinates and facilitates Enterprise Root Cause Analysis (SWARMS) of incident reports. Maintains data for weekly and monthly reporting.

Recognition Program - 5% Supports the development of a recognition program aimed at improving patient safety and leads in the administration of the Patient Safety Culture survey.

Risk Assessments - 5% Based on identified risks, gives recommendations on processes for conducting proactive risk assessments (FMEA) to the patient safety committee.

Patient Safety Education - 5% Design and implement educational presentations that facilitate the understanding and implementation of patient safety standard within the organization on an ongoing basis.

Patient Safety Program Review - 5% Assist in the creation, review, and refinement of the scope of the Patient Safety Program within the facility on an annual basis.

Qualifications

Minimum Required: RN Associate's Degree

Preferred: RN Bachelor's Degree

Minimum Required: Nursing Diploma | Registered Nurse |

Minimum Required: 3 - 5 Years equivalent experience.
