Minimum Data Set Registered Nurse (RN), Day Shift, Skilled Nursing Facility (SNF)
UC Health is hiring a part-time RN for SNF, for the first shift at Daniel Drake Center for Post-Acute Care.
The MDS RN will support Daniel Drake's SNF department, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures.

Responsibilities
The Minimum Data Set (MDS) Nurse coordinates and completes the Resident Assessment Instrument (RAI) and restorative plans of care in a timely and accurate manner. The MDS Nurse ensures that documentation is in compliance with federal long term care regulations, Prospective Payment System (PPS) requirements, and facility/departmental policies, procedure and rules. The MDS Nurse reviews documentation to monitor that the care plan is being followed and to ensure that staff is providing care that reflects the wishes of the resident.
The MDS Nurse establishes restorative programs to enhance the resident's quality of life, improve or maintain functional status, and optimize case mix reimbursement as appropriate. The MDS nurse plays an active role in educating staff and interviewing residents according to MDS needs. The MDS nurse works closely with therapy and nursing team to maximize RUG scores.

- Collects, organizes and analyzes data about the health status of individual residents to accurately reflect resident functional status, strengths, weaknesses and preferences
- Assesses clinical information to establish optimal ARD according to regulatory and reimbursement requirements. Established appropriate RUG category based on clinical findings and supporting documentation.
- Coordinates and tracks completion of triggered CAA's assigned to nursing and other disciplines.
- Communicates goal and intervention information and incorporates interdisciplinary team suggestions into a plan of care as appropriate.
- Reviews Assessment Reference Dates (ARDs) with therapy staff to coordinate the delivery of therapy minutes with MDS observation periods.
- Maintains documentation as required by regulatory bodies and assists with regulatory surveys including ODH, Advancing Excellence, Certs, JCAHO and others.
- Completes assigned sections of the MDS accurately, utilizing critical thinking skills, sound nursing judgment and knowledge of Quality Measures/Indicators, according to RAI manual guidelines.
- Coordinates completion of CAA's assigned to other disciplines per facility policy and procedure
Qualifications
- Graduate from an accredited school of nursing
- Associate's Degree, minimum required
- Bachelor of Science in Nursing, preferred
- Current OH RN License
- Minimum Required: 3 - 5 Years equivalent experience
