The Population Health Program Coordinator is responsible for assisting the Population Health Team with managing our Value Based Program portals, abstracting patient charts and closing identified health gaps. The Program Coordinator must have the ability to communicate effectively to all levels of the organization, and work collaboratively with associates, physicians, vendors, and external customers to promote a positive environment.
Responsibilities
Communication: • Review yearly Payor and CMS Guidelines to determine Measure Year Goals, Incentive amounts, and documentation requirements. • Attend Payor meetings to discuss metrics, goals, and incentive programs. • Communicate with Payors to discuss supplemental documentation for Gap Closure. • Keep management and executive team informed about the status and expected completion of incentives. Portal Management: • Access Payor portals to construct an open measures list. • Utilize Payor portal training for plans, payors, incentive goals, and submission requirements. • Confirm supplemental data submission and current metrics through Payor portals. • Regularly review the Payor portal to ensure the program is on target for maximum incentive return. Daily Tasks: • Abstract charts to confirm and download supplemental documentation. • Access the Release of Information to create supplemental documentation for measure closure. • Confirm submission requirements for each payer (e.g., portal upload requirements, fax, or email). • Confirm patient attribution and construct a list for review by providers. • Create and maintain an Excel spreadsheet to track updates on attributed patients for use in chart abstraction. • Utilize the spreadsheet to generate actionable data for creating metrics for Incentive program.
Maintain confidentiality of patient and employee information, initiate and develop positive relationships with associates, management, and health care providers, exhibiting respect, flexibility and cooperation at all times. Communicate in a way that conveys appropriate empathy and compassion; utilize department and hospital resources to achieve positive outcomes. Assist in the interpretation of policies and procedures to staff. Assist Department Leader in updating policies, maintaining preventative maintenance paperwork and databases, and ensuring regulatory documentation is updated in a timely manner. Assist leadership with any regulatory body visits to the healthcare facility. Serves as a positive role model in representing the department and UC Health values, policies and procedures. Takes the initiative in identifying and facilitating the resolution of administrative and departmental problems through research, communication and cooperation with internal and external personnel. Manages the payroll process for the department staff members. Other duties as requested and assigned.
Qualifications
Minimum Required: High School Diploma or GED; Preferred: Associate's Degree | Minimum Required: 3 - 5 Years equivalent experience; Preferred: 6 - 10 Years equivalent experience.
