The Project Manager for Population Health will be responsible for working with a cross-functional group of stakeholders across Ambulatory and UCPC to manage the overall portfolio of value-based programs and payor contracts across Ambulatory and UCPC. Key functions of this role include but are not limited to: Project Portfolio Manager
  • Ensuring UC Health is in compliance with program requirements
  • Identify opportunities for improvement on program metrics and partner with clinical teams, revenue cycle, payor relations, IS&T/Epic, etc. on associated improvement projects
  • Reporting of program progress, results and financial updates to leadership and develop recommendation on next steps for program engagement
  • Serve as the point of contact for payor partnerships and serve as the subject matter expert for utilizing relevant payor portals
  • Plan, organize, coordinate, direct and control all aspects of projects/tasks as they relate to relevant value based program


Responsibilities

The Senior Project Manager for Population Health should bring a mix of experience, talent, engagement, and values that will enable success in a complex and diverse healthcare environment. A successful candidate with have excellent communication skills, including writing, speaking and facilitating and problem solving in groups. The excel in this positing a working understanding and willingness to learn more about payor partnerships, HEDIS metrics, CMS regulations, CPC, etc. is key

Job Function: Project/Program Management -

Development of project timelines to monitor progress on project milestones. Appropriate escalation when projects are at risk of not meeting deadlines.

Tasks, staffing requirements, interdependencies, risks

Action plans to course correct when projects are off track

Manage the creation of work products and projects of others to ensure quality deliverables while working independently and on teams.

Manage performance requirements and escalate issues to appropriate leaders.

Facilitate and manage presentations, meetings, and working sessions.

Maintain files/file sharing, set meeting agendas, meeting minutes, and management of action items.

Job Function: Finance Management -

Partner with the internal finance team to develop reports for tracking revenue and expense.

Tracking of payments coming into the system as a result of value based programs and payor contracts.

Reconciliation of funds expected vs. actual funds received.

Regular updates to department leadership focused on the financial performance and anticipated risks of not meeting program financial projections.

Job Function: Stakeholder Communication Engagement -

Partner with Clinical Operations, Billing/Coding, Accreditation, Revenue Cycle, Care Coordination, IS&T, Payor Relations, and numerous other teams across the system to support smooth end-to-end management of programs and payor contracts.

Regular communication and updates to leadership teams and stakeholder group to maintain clarity and collaboration across programs.

Attendance at payor meetings and summarize key takeaways and recommendations for Ambulatory and UCPC to make decisions.

Job Function: Performance Improvement -

Leverage the UC Health PI Way to lead VBP improvement projects.

Project charter, process maps, outcome/process/balancing metrics, failure mode identification, etc.

Job Function: Data Monitoring & Analysis -

Review of data available from internal and external sources.

Identify gaps in data and partner with internal and external teams to develop reports and dashboards to fill said gaps.

Data analysis to support data drive decision making

Job Function: Other Duties -

Other duties as assigned that align with the Ambulatory/UCPC strategy and goals as it relates to the Population Health portfolio of work

Qualifications

Bachelor's Degree in Health Sciences, Healthcare Administration, Nursing or a related field with relative healthcare experience

Master's Degree - Preferred

3 - 5 Years relevant experience - Minimum Required
