Transfer Center Supervisor

Job Description

Provide operational supervision and coordination of multiple activities for areas of Bed Control, Transfer and Physican Referral Center (TPRC), Scheduling and Census. Coordinate TPRC marketing visits and maintain communication/correspondence with outside hospitals. Serve as physician contact for TPRC concerns. Perform and track TPRC improvement metrics.

Responsibilities

Provide supervision for Bed Control, Transfer and Physician Referral Center Scheduling and Census. Encompasses problem identification analysis and resolution with staff and the appropriate bed placement for patients. Serve as Bed Control representative to relevant ad hoc committees. Ensure Access Coordinator perform daily tasks. Provide placement assistance as needed.

Coordinate TPRC activities to include marketing visits and maintain communication/correspondence with outside hospitals. Develop TPRC marketing materials in collaboration with Public Relations & Marketing Department. Serve as physician contact for TPRC concerns. Serve as TPRC representative to relevant ad hoc committees. Perform and track TPRC improvement metrics. Maintains database of all requests & outcomes for TPRC program and development.

Review Bed Control staff work utilizing the Quality Review Form on a weekly (or as needed) basis to ensure that patient placements, transfers and registration activities are handled appropriately and to ensure data integrity. Provide timely feedback to Bed Control staff after conducting a Quality Review. Provide on-going communication and direction to team members through staff meeting and e-mail, with the ability to accept and implement change.

Schedules, determines work priorities and assigns work to personnel, based on knowledge of goals, objectives and individual capabilities. Ensures timely and accurate completion of work activities. Develops staffing strategies and implements schedules that are consistent with meeting the budget objectives. Uses supplies and staff resources economically. Available to staff via phone and pager for problems.

Maintain understanding of the following: Total discharges for the facility, total bed requests for the facility, high risk areas (CEC, ICU's, etc.) Assist in preparing the daily census as needed. Review the following in the online Patient ONTRAC tool: Service Access Reports, Redirect/Cancellation/Delay Detail Report. Follow up with Bed Control staff on report concerns as needed. Provide updates to the Director prior to the weekly Patient Progression meeting. Attends weekly Patient Progression meetings. -

Establish and maintain good interpersonal relationships. Provide communication to physicians, administrators, clinical managers, nursing supervisor and nursing staff regarding new issues and resolution of problems and difficult situations. Meet as needed with the Director to review Key Performance Indicators (KPI), staff issues, Quality Review scores and issues that require elevation to the Director. Liaison between Admitting, Center for Emergency Care and members of other departments. -

Qualifications

Minimum Required: Associates degree combination of education. 3-5 Years equivalent experience or leadership experience in healthcare setting

Preferred: Bachelor's and/or master's degree.
