UC Health is hiring a Full time Diabetes Educator for the Endocrinology Department at University of Cincinnati Medical Center.
The Diabetes Educator will support the Medical Center, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures.

Responsibilities
The Diabetes Educator works collaboratively with the Diabetes/Endocrinology team and the specific population served to provide discipline and level appropriate clinical and educational services. The focus of the position is transmitting knowledge related to essential skills for safe self-management to referred patients, family members and caregivers. This position will support the recognition/accreditation of the UC Heath Diabetes Self-Management Education and Support program through adherence to program requirements, collection and reporting of program data, and participation in quality improvement initiatives.

Responsibilities
The Diabetes Educator works collaboratively with the Diabetes/Endocrinology team and the specific population served to provide discipline and level appropriate clinical and educational services. The focus of the position is transmitting knowledge related to essential skills for safe self-management to referred patients, family members and caregivers. This position will support the recognition/accreditation of the UC Heath Diabetes Self-Management Education and Support program through adherence to program requirements, collection and reporting of program data, and participation in quality improvement initiatives.
- Has knowledge of growth and development milestones and tasks.
- Interprets population-related data and plans care appropriately.
- The scope of the education consultant is primarily at the hospital/corporate level.
- The Education Consultant is responsible for the development, implementation, and evaluation of the education plans through the use of education needs assessments as needed, guidance from interprofessional leadership, and current evidence.
- Applies design principles and integrates technology to ensure effective learning experiences for competency assessment and validation to reflect practice changes, regulatory agency, or organizational policy requirements.
- Serves in collaboration with nursing and interprofessional leadership, the Professional Practice Organization (PPO), and other UC Health entities to implement nursing education and professional development programs.
Qualifications
- Minimum Required: Associate's Degree - If Registered Nurse (RN)
- Minimum Required: Bachelor's Degree - If Registered Dietitian (RD)
- Active Registered Nurse or Active Registered Dietitian
- 0-6 months of general clinical or chronic care experience specific to discipline (RN, RD)
- 0-6 months experience providing care, support, and education to diabetes patient population to assure essential skills for safe self-management.
