The Manager of Care Management is responsible for the development and management of the infrastructures that support the effective placement, transition, ongoing monitoring of services, the final disposition of every patient treated at University Hospital, and the 24 hour accountability of the Center for Emergency Care. The Manager is responsible for developing and implementing quality review programs, key performance indicators, and the care management activities of patient progression. When escalated from case managers and/or social workers the Manager interacts with Medical, Nursing, and Executive leadership to facilitate quality based, cost effective, patient outcomes that ensure care is provided in the most appropriate setting. The Manager communicates best practices, current knowledge, and expectations of staff as related to social work, nurses, physicians, payers and other healthcare team members. The Manager is responsible for budgeting, assists with personnel recruitment, retention, corrective action and professional development.

Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs that promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families' emotional, psychological, social, financial and developmental aspects of care.

Engagement and Exploration:
  • Manage, supervise, and monitor activities of supervisors, Masters prepared social workers, support staff; and social work students in assigned areas plan, coordinate, and implement programmatic changes that enhance improved patient care.
  • Develop, manage, and evaluate programs and services. Participate in activities that monitor, refine, and promote efficient patient throughput.
  • Remove barriers that impact patient care; escalate system issues up through Director, VPs and the CEO. Promote inter and intra-department communications.
  • Establish and maintain a working environment that supports the RITE Values. Motivate staff to achieve highest level of professional and personal development.
  • Develop and implement departmental policies and procedures.
  • Ensure associates know performance expectations and hold them accountable.
  • Communicate and convey information to staff that promotes staff involvement and satisfaction.
  • Develop and implement education programs for social workers and other multidisciplinary hospital staff; provide instruction and consultation to medical students and residents.
  • Coordinate social work student internships.
  • Provide an environment that utilizes the core values of standards of practice and ethics.
  • Develop and maintain liaison relationships with community agencies, extended care facilities and funeral homes. Provide direct patient care services as an integral part of the escalation process.

Goal Development and Implementation:
  • Participate in weekly patient and associate rounding activities and prepare summary reports.
  • Participate in activities that promote quality and safety. Implement strategies to enhance productivity and quality of work.
  • Participate in hospital quality improvement teams, committees, and special projects. Actively participate in the department Continuous Quality Improvement (CQI) program.
  • Work with medical staff, department managers, and multidisciplinary hospital staff to problem-solve and facilitate effective service delivery.
  • Develop methods and measuring devices that educate staff and improve the quality of documentation.
  • Recruit, interview and select associates for hire Reduce turnover and vacancy rate within area of responsibility.
  • Participate in activities that support staff retention.

Evaluation and Termination:
  • Demonstrate competency in addressing the unique age related physical, psychosocial and social needs of patients across the life span.
  • Promote behavioral standards that support the values and ethics of the social work profession.
  • Provide social work supervision to staff to promote patient/family satisfaction.
  • Troubleshoot and mediate patient/family issues with social workers.

Education and Consultation:
  • Provide consultation and training to medical staff and other healthcare professionals on psychosocial issues regarding illness and barriers to maximizing improved health status.
  • Collaborate with co-workers and management to ensure the on-going development of the Social Work Department.


Master's degree preferred. MSW, LISW- S Required. Licensed Master prepared Social Worker Certification in Case Management strongly preferred.

Five (5) years combination of clinical nursing and/or social work, care management, and leadership experience in an acute care setting is expected.

Experience in level of care management and discharge planning is essential. Third party administration also preferred.
