The Program Coordinator is responsible for assisting the Department Leader with daily department administrative functions. The Program Coordinator is required to be proactive in the development of business practices and plans that improve operations and cost effectiveness, may assist other management personnel in the department as well as be responsible for payroll and department records. Must have the ability to communicate effectively to all levels of the organization, and work collaboratively with associates, physicians, vendors, and external customers to promote a positive environment.

Responsibilities

Description for Internal Candidates
Responsible for establishing trusting relationships with women and their families while providing general support and encouragement.

Interpersonal skills that foster trust, including sensitivity, empathy, flexibility, accessibility, patience, resourcefulness, enthusiasm, and the ability to be non-judgemental

Conduct intake interviews with women including use of ODH-identified screening tool

Develop and maintain a portfolio of local referral resources to ODH-funded services, evidence-based interventions, clinical and social services.

Connect and refer women with applicable programs and services

Provide follow-up to women via phone calls and other means as necessary to ensure her needs are met.

Be knowledgeable about community resources appropriate to needs of women/families

Document woman/families referral and addressed risk status via the ODH-identified platform

Exhibit excellent working relations with women, visitors and staff

Coordinate with Ohio Equity Institute Project Coordinator to review documented barriers and access challenges
Responsibilities for Internal Candidates
Maintain Departmental Standards
  • Establish, maintain, and audit patient satisfaction information and data to improve patient satisfaction initiatives
  • Investigate patient satisfaction discrepancies and make recommendations to resolve and ensure future discrepancy is not repeated
  • Audit and analyze departmental processes and systems to ensure accuracy and all tasks are being carried out appropriately
  • Act as Subject Matter Expert for department within scope of practice
  • Maintain department goals and or tasks to keep them on schedule and help implement them
  • Update departmental policies and procedures, maintain records, and distribute to staff

Administrative Support
  • Maintain personnel file system
  • Maintain calendars
  • Document controAssist with copying, faxing, and mailings related to your patients
  • Create reports to review and maintain enterprise standards
  • Draft various documents and maintain departmental manuals/rosters/lists


Other Duties as Assigned

Qualifications

Minimum Required: experience working in a community-based setting for at least 1 to 2 years preferred

Experience working in a multi-cultural setting

Basic computer skills required

Good Communication skills, such as listening well and using language appropriately

Understand the community served-community connectedness.
