The position involves patient assessment and triage, care coordination and navigation, and community education and outreach.
Responsibilities
Esophageal Disease Center Coordinator
- Manages new referrals, obtaining all external records and imaging and schedules all appointments in the clinic
- Coordinates all care including scheduling tests, referrals, follow up appointments and surgery
- Educates patients and families
- Facilitates transition of care from inpatient to outpatient, including ensuring home health care, wound vacs, tube feed supplies, etc are arranged
- Coordinates esophageal tumor board and EDC center meetings
- Identifies system issues that may impede attainment of outcomes and outline potential strategies for resolution
- Develops and evaluates critical pathways in collaboration with the multidisciplinary team
- Thoracic Surgery Patient Coordinator
- Supports thoracic surgery clinics
- Provides preoperative education for all surgical patients and serves as continuity between in-patient and out-patient care
- Handles post-discharge patient calls to ensure appropriate progress
- Obtains consents for thoracic tumor bank and database
- Handles all medication refills/new prescriptions, including prior authorization
- Assists with coordinating care, such as obtaining cardiology clearance
- Assists with scheduling surgeries, procedures, etc.
- Triage all patient calls
- Serves as the primary resource for patient calls, properly triaging medical urgency
- Works with provider to establish patients' next step of care regarding their clinical concern
- Handles all medical refills, patient MyChart advise requests, and prior authorizations
- pathways in collaboration with the multidisciplinary team
Lung Volume Reduction Surgery
- Manages new referrals, obtaining all external records and imaging
- Helps coordinate care for preoperative patients, including providing patient/family education
- Facilitates transition of care from inpatient to outpatient and coordinates long-term follow up including administration of required surveys
- Maintains LVRS database as required for CMS certification
- Coordinates Joint Commission certification for LVRS, including preparing documents for site visits
- Identifies system issues that may impede attainment of outcomes and outline potential strategies for resolution
- Develops and evaluates critical pathways in collaboration with the multidisciplinary team
Thoracic Surgery
- Supports thoracic surgery clinics
- Provides preoperative education for all surgical patients and serves as continuity between in-patient and out-patient care

Qualifications
- Minimum Required: Bachelor's Degree - Nursing.
- Preferred: Master's Degree - Nursing.
- Licensed to practice as a Registered Nurse in the State of Ohio.
- Current BLS required. ACLS certification Preferred.
- Certification in specialty area strongly preferred.
- Minimum Required: 1 - 2 Years equivalent experience.
- Preferred: 3 - 5 Years equivalent experience.
