Senior Health Unit Coordinator, LTAC 3 North, 1st Shift

Job Description

Provides the clerical support to the clinical team in carrying out the daily functions of patient care. This position provides the pivotal communication link for processing information/requests to and from patients, staff, visitors, physicians, and other departments or agencies. Has responsibility for initiating and/or ensuring the data entry of patient related functions/processes, as well as the management (maintenance) of the medical record and its related forms, documents, from admission to discharge. Communication is culturally based and age-specific.

Monitor Watcher: Cardiac monitoring duties include observation, alerting nursing staff of changes in cardiac rhythm, providing ECG strip of cardiac rhythm for documentation as scheduled per policy and changing electrodes/batteries in telemetry transmitters as needed.

Responsibilities

Chart Maintenance: - 15% Places forms and test results in charts in a timely manner Copies pieces of medical records and retrieves as necessary Keeps all charts in designated order Fills charts as needed (admit notices, Continuity of Care, admission paperwork, consults) Prepare and mail out IDC letters; file copy of letter and give copy to resident; other letters as requested Conducts audits to support the unit with regulatory compliance and process/quality improvement. Specific data will be identified and modified according to regulatory and unit needs. � Communication: - 30% Keeps Clinical Manager/staff informed of issues Meets and greets patients, visitors and staff in a warm, courteous manner Answers patient call lights and unit phone promptly Uses nurse call system appropriately Ensures that admission forms are signed, as needed Facilitates communication with ancillary departments Communicates with various departments inside the hospital in a professional, cooperative manner Serves as a resource for location of supplies and medical records Adheres to all departmental and hospital customer service standards Performs courier duties as directed, including proper use of the pneumatic tube system Retrieves information from the computer Reviews Continuity of Care and admitting paperwork to identify any follow-up appointments as needed Schedules appointments, including transportation, internally and externally Schedules consults Completes appointment log Pages physicians as needed Informs RN of squad arrival Updates white boards in patient rooms daily by 9:00am and 9:00pm Communicates menu changes to patients/residents Maintains admit and discharge logs Communicates room changes and care needs to families and agencies Follows up on diabetic shoe referrals Round and maintain log for contracted services (podiatrist, dentist, optometrist) Assist with Unit team needs Coordinate unit collaborative efforts that foster positive environment and team development Assist with culture change efforts within the team and promote positive tools that assist staff with change. Prepare and mail out IDC letters/file copy of letter and give copies to appropriate persons Act as back up to verify insurance approvals � Computer Responsibilities: - 15% Admits patients in a timely manner Transfers and discharges patients in a timely manner Enters charges/reconciles RMS report with daily changes Maintains a manual order entry system when computer is down; prepare requisitions for services Proficient in EPIC computer program, including transfer and discharge of patients Prints necessary paperwork (i.e. Ticket to Ride, face sheet, History & Physical) Prints discharge paperwork Prints arm bands Prints labels and distribute to RNs Programs phones and maintains phone log, which will be provided to Unit Manager daily. Inventories phones and batteries monthly and provides counts to Unit Manager. Updates census and fax to House Supervisor Pulls assignment sheets, admission faxes and Continuity of Care Audits weight book for weekly weights, variation and documentation in EPIC. Notifies Unit Manager of discrepancies or missing weights. Posts staffing sheet in preparation for Surveyors Audits shower documentation in EPIC Telemetry: ��� Documents and interprets rhythm strips correctly on medical record ��� Documents clinical event trends as necessary ��� Continuously observes cardiac monitors for patient dysrhythmias and reports to RN ��� Takes monitor strips to RNs to review � Environment/Supplies/Equipment: - 10% Uses fax, copier, printer Routine maintenance for fax, copier, printer Orders supplies in a manner that considers departmental needs and budgetary limitations resulting in a consistent monthly cost Verifies and puts away supplies and equipment Types and files for unit Calls 585-MYPC to open tickets, as requested Restocks and straightens nurse servers Other duties as delegated by RN Shops for clothing and other items as needed; assists with Medicaid spend down � Unit Responsibilities/Accountability: - 30% Serves as a positive role model � maintains a positive attitude Participates in unit counsel activities Responds to scheduling responsibilities and with timeliness Contributes to unit performance improvement activities Attends in-services and unit meetings Takes workshop residents to and from bus Collates and puts MDS raw data into a user friendly form Assists with patient satisfaction initiatives Collate and provide call light response time information for management follow up Coordinates extended care unit workshop Assist with responding to needs for TCU and notifying appropriate staff for follow up Assist with volunteer development program Assist with the approving and auditing of Payroll Send out letters regarding resident needs or requested by administration/families/residents Communication with families when room changes are needed on ECU Order needed office supplies for unit Assistance with development of processes that will facilitate efficiency and consistency on unit Will coordinate schedule for nursing staff Will track time and attendance | Review the physical demands and assign an overall average percentage of time this position performs these activities.
